Job Description
ADA Accredited Diabetes Center seeks Advanced Practice Provider-
 Northampton, MA

Cooley Dickinson Health Care, a member of Mass General Brigham, is currently seeking a Nurse Practitioner or Physician Assistant to join our physician and nurse educators in our Diabetes Center to provide care to those with Type I, Type II and Gestational Diabetes.

The Diabetes Center is located in our beautiful, Northampton-based, medical office building along with primary care, OBGYN, and specialty practices.
We are an American Diabetes Association-recognized Diabetes Education program. Services include diabetes self-management education program, diabetes medical management, medical nutrition therapy, and insulin pump therapy. We also offer a handful of classes for patients living with or recently diagnosed with diabetes. 

This position will be expected to practice as an independent practitioner, in a team-based care model. Our ideal candidate will have at least six months of experience as an advanced practice provider, ideally in a specialty, diabetes or endocrinology setting. The candidate should be comfortable with diabetes medication titration, insulin pumps, and have prior experience working with diabetes patients.

Candidate must have graduated from an accredited institution as a Nurse Practitioner or Physician Assistant. Current Massachusetts licensure is strongly preferred.

The Cooley Dickinson Health Care system, an affiliate of Massachusetts General Hospital and Mass General Brigham, has a medical staff of approximately 500 active members (physicians and advanced practice clinicians). The system is comprised of the Cooley Dickinson Hospital, Cooley Dickinson Medical Group, as well as a VNA and Hospice.
